It's been reported to me that the QC importer is stalling in 2013 and the weight issues are incorrect on models imported in 2013. I've just verified the crash on Max 2013.
I will look into this when I get some time. At the moment, I suggest using Max 2014+.
I took a little time to look at this and try to work out the problem for 3ds Max 2013... but I could not quickly figure it out. I've updated the docs to reflect this limitation and reiterate that you should use Max 2014+. I will try to fix this when I find time.
